Depending on your insurance policy, you may qualify for direct access to services or need a referral from your primary care physician. If a referral is needed, ask your primary care physician to refer you to Therapy Solutions. Once the referral is made, please contact us to make an appointment.
Therapy Solutions is in-network with most major health insurance plans including Medicare. You may want to contact your provider prior to scheduling an appointment.
Before Your First Visit:
Client forms are available on our website. These can be printed and filled out prior to the appointment to the best of your ability. If you are unable to complete the client forms before your appointment, please ensure you arrive at least 20 minutes early to fill them out in the clinic.
Bring your insurance card or a copy of the front and back of your insurance card. If you have more than one insurance provider we will need all insurance information.
If you do not have insurance coverage, you will be considered a self-pay client, with a cost of $100 per hour.
If your insurance requires a primary care physician referral, please contact your primary care physician to have a referral sent to Therapy Solutions. You may call and check that we have received the proper referral or bring to your first appointment.
Prior to your first appointment, print and fill out the new client forms below to the best of your ability. Please bring all the forms to your appointment. If you are unable to complete the forms prior to your appointment, please arrive at least 20 minutes early to ensure you have adequate time to complete the forms before your appointment time.
What to expect at your first appointment and throughout treatment:
An assessment and evaluation is the first stage. This includes medical history, injury, current pain, health history and overall health assessment. Your therapist conducts a thorough physical examination to assess range of motion, pain levels, strength and ability to perform specific functional tasks.
Using the assessment and evaluation information, you and your therapists discuss and establish therapy goals and a treatment plan.
Your therapist will recommend and discuss with you an individualized treatment plan, which may include a program at the clinic and home exercises.
We encourage people to ask questions during the course of therapy.
What to wear:
We ask that you wear comfortable, loose fitting clothing; something that allows your therapist to access the treatment site.
What can you do?
- Active participation is essential toward achieving your goals. We recommend the following to increase the ability to reach your goals.
- Actively participate during the treatment by following through on all home exercise programs prescribed by your therapist.
- Openly communicate with your therapist and report any significant changes in symptoms, medications and function.
- Keep scheduled appointments and arrive on time to provide maximum time with your therapist.
